THE MINISTRY OF HEALTH EXPLAINS NEW APPROACHES TO THE FORMATION OF A SINGLE PACKAGE OF MEDICAL CARE 24.12.2024
The Ministry of Health is proposing a phased transition to a Single Package, which involves the inclusion of primary health care (PHC) services in the MHI package by 2027.At the same time, free state guarantees (GOBMP) for all citizens, regardless of the status of insurance in the CSHI, will remain for the following types of medical care::- Ambulance service;- Diagnosis and treatment of all infectious diseases;- Diagnosis, treatment, drug provision, inpatient and inpatient replacement care for socially significant diseases that cause the greatest damage to the health of citizens (tuberculosis, mental and behavioral disorders, HIV, oncology, chronic hepatitis and cirrhosis of the liver, orphan diseases, acute myocardial infarction, strokes);- Emergency inpatient care for life-threatening conditions;- Palliative care;- Provision of blood products. The implementation of the proposed Single Package of Medical Care will lead to the following positive changes. These are the clarity of the boundaries of the GOBMP and the CSHI, without duplication of services, the availability of examinations for suspected socially significant diseases and the passage of cancer screenings, regardless of the status in the CSHI, and an increase in tariffs in accordance with real costs. It is worth noting that in the insurance model, the state will continue to make contributions for 15 categories of beneficiaries, which include children, pregnant women, pensioners, students, the unemployed, people with disabilities and many other categories of the population. Therefore, these socially vulnerable categories, which amount to over 11.9 million Kazakhstanis, are protected by the state and will have access to medical care at all levels, including primary health care services. Citizens of working age insured in the CSHI system, which is 5 million people, also have full access to medical care.In addition, over 1 million citizens of working age who belong to crisis (E) and emergency (E) levels of social well-being, that is, low-income, will be among those insured in the CSHI system at the expense of local executive bodies from 2026 and will be able to freely receive additional medical care in the CSHI package. The remaining categories belonging to category A, B, C according to the digital card of the family of the Ministry of Health, those with sufficient income will have to insure themselves in order to receive medical care.As a result of the inclusion of these categories of citizens in the CSHI system, the level of insurance in the group of people with socially significant diseases will increase to 93%.An important example is international practices where PHC is already included in insurance packages. For example, a single package of basic state medical care includes: in Russia, medical aviation, HIV/AIDS, sexually transmitted infections, tuberculosis and mental illnesses; in Estonia – emergency medical care, HIV/AIDS, infections, tuberculosis, and mental illness; In Lithuania, only emergency medical care is provided.; In Germany and the Netherlands, only nursing care is provided.Kazakhstan, in turn, has included services in a Single package of basic state medical care, the number of which significantly exceeds services in foreign countries. Thus, the measures taken to increase the coverage of the population with health insurance and the state's obligations to insure the privileged category of the population minimize the risks of unavailability of medical care, including the provision of primary health care. These standards are published on the portal of open NPAs for public discussion within the framework of the draft Law of the Republic of Kazakhstan "On Amendments and additions to certain legislative acts on compulsory social health insurance".Source : https://www.gov.kz/memleket/entities/dsm/press/news/details/906585
